River’s Journey Counseling is committed to maintaining past and current client’s confidentiality. We will only release healthcare information about you in accordance with federal and state laws and ethics of the counseling profession.
This notice briefly describes our policies related to the use and disclosure of your healthcare information. Upon entering into a counseling relationship at River’s Journey Counseling all clients are also provided with a copy of the detailed HIPAA Notice of Privacy Practices.
Uses and disclosures of your health information for the purposes of providing services.The following uses and disclosures of PHI will be made only with your (or authorized representative’s, such as a parent) written authorization: Providing counseling related services, collecting payment and conducting other healthcare operations are necessary activities for quality care. State and Federal laws allows us to use and disclose your health information for these purposes.
Counseling We may need to use or disclose health information about you to provide, manage or coordinate your care or related services. Which could include consultants and potential referral sources.
Payment Information needed to verify insurance coverage and/or benefits with your insurance carrier, to process your claims as well as information needed for billing and collection purposes. We may bill the person in your family who pays for your insurance. At your request, we may not disclose treatment information to your health insurance carrier for which you have paid out-of-pocket, unless the disclosure is required by law.
Healthcare Operations We may need to use information about you to review our treatment procedures and business activity. Information may be used for certification, compliance and licensing activities. River’s Journey Counseling does not use and/or disclose PHI for marketing purposes. River’s Journey Counseling does not sell PHI. River’s Journey Counseling does not send fundraising communications to current or former clients. However, these activities do require a client’s signed permission and ability to choose to opt out of activity.
Former and active clients of River’s Journey Counseling will be notified if there is a case of any breaches of unsecured PHI.
Other uses or disclosures of your information which do not require your consent. There are some instances where we may be legally and ethically required to use and disclose information about you without your consent to keep you or another person safe. For example, but not limited to: Information regarding child abuse or elder abuse (or a dependent adult) would be reported to the proper authorities; such as the Department of Children and Family Services, in cases involving suspected child abuse. Additionally, if there is a specific report or threat that you are in danger of harming yourself or others. We must notify the police and attempt to warn the person you intend to harm. If you have verbalized intent to harm yourself, we will try to work with you to develop a plan to keep you safe. If we are unable to develop a plan, we will contact your emergency contacts and the police to try to keep you safe. Lastly, information shared with law enforcement if a crime is committed on our premises or against our staff or as required by law such as a subpoena and/ or court order.
